You have been selected for admission to the 1st year MBBS class for the session 2012-20 13 in

This University, purely on provisional basis subject to the following terms and conditions:-

a. You will be medically examined before your fee and documents are accepted.

b. You have to submit the following documents in original and (4) four attested photocopies of each with the

University office before depositing the University Fee.

I. Secondary School Certificate (Matric/equivalent Certificate).

2. Certificate/detailed marks certificate of F.Sc. (pre-medical/equivalent).

3. Provisional Certificate/Character certificate of college last attended.

4. Result card of Entry Test.

5. Domicile Certificate.

6. Board migration certificate in case of the Board of F.Sc. is other than Lahore.

7. Birth certificate if date of birth is not available on Matric certificate.

8. Four passport size photographs (sky blue background) duly attested by a Govt. officer of grade 17 or above.

9. Certificate that you have been inoculated against the enteric group of fevers within the preceding 12 months.

I 0. Certificate that you have been fully vaccinated against Tetanus.

II. Certificate that you have had a full course of immunization against Hepatitis 'B' Virus.

12. University fee for the session 2012-2013 is Rs.23880/-

13. Surety Bond (specimen enclosed) on a stamped paper of worth Rs.100/ - duly attested by the

Judicial Magistrate.

14. PM&DC undergraduate student Registration Form (enclosed).

c. Your admission is liable to be cancelled at any time during the course of study if any of your documents, certificates or statement is found wrong in any way at any stage. Admission is also liable to be cancelled/changed if any error or omission is detected at any stage.

d. Apart from this, all the rules and regulations contained in the Prospectus of the Govt.

Medical Colleges of the Punjab session 2012-2013 and changes from time to time, are

Applicable, and they are binding on you.

If you accept, please report to the office of the undersigned along with this call letter, fee and documents mentioned above on 06-07 and 08.12.2012. If you fail to pay the fee along with required documents and certificate within the stipulated date, your selection will be cancelled. In case you are not willing to accept this offer, please submit your reply in writing to this office within the target date. Time to report for medical examination and deposition of fee/documents is 8.30. A.M. sharp.
